Class of Housing/Services: ransitional housing for homeless men and women with Mental health issues, substance abuse issues and HIV/AIDS.

General Information:
House of grace is a structure transitional housing for homeless men and women with Mental health issues, substance abuse issues and HIV/AIDS. Clients must meet at least two of the admission of the criteria. Must have income and Medicaid. Must be willing and open- mined to a new way of living. The House of Grace offers a Transitional Housing Program for men and women with HIV/AIDS, substance abuse issues and mental illness. The House of Grace also offers a Women’s Reunification Program for women and children. These programs last from 1 to 2 years. The mission of the House of Grace is to provide a safe and supportive environment while improving the quality of life for homeless single men and women with substance abuse issues who are living with HIV/AIDS. The House of Grace, Inc. offers the following Substance Abuse classes and therapy groups:     Relapse prevention 32 modules (16 Week), certificate given after completion     Aftercare group weekly     Women’s S.A. Issue therapy group     Men’s S.A. issue therapy group Admission Criteria:     Must be medically certified HIV positive.     Must be Medicaid eligible in North Carolina.     Must be in need of case management services.     Must be willing to actively participate in a Case Management Plan.

Transitional housing is affordable supportive housing designed to provide housing and appropriate support services to persons who are homeless or who are close to homelessness. The transition is to help them be more self sufficient to move towards independent living on their own. Transitional housing locations are oftentimes called Halfway Houses. The naming of the two (Halfway House OR Transitional House) is oftentimes interchangeable.

Many Transitional Housing programs are subsidized and are income based. Income based transitional housing generally charges 30% of your income for rent when you do have income. Contact the transitional housing facility to see if this applies.

Services provided at transitional housing facilities varies from substance abuse treatment, to psychological assistance, job training, domestic violence assistance, etc. The assistance provided varies, but it is generally affordable and low cost housing. Read the descriptions of each of the transitional living locations for more detailed information. Transitional housing facilities also may or may not assist with alcohol and/or drug substance abuse treatment.
