General Information:
METAMORPHOSIS provides Shared Housing for females, Assistance with Activities of Daily Living, Housing Readiness Training,Case Management, Medical Services Assistance, and Mental Health Referrals Services: 24 hour seven days a week assisted living, on-site staff who are experienced in working with intellectual and developmental disabilities, meal planning and preparation, shopping, house cleaning and grounds maintenance, transportation to medical facilities, grocery stores, shopping centers, recreational activities, assistance with medical needs and medication administration, supervised in-house activities, field trips to sporting and cultural events, life-learning skills: hygiene, self-help care, communication  skills, socialization, community safety skills, problem-solving,  decision-making, table etiquette, minial work skills, money management, personal  laundry and housekeeping. Eligibility Criteria Those eligible for services are women ages 18 and  older who are unable to care for themselves, but do  not need intensive nursing health and/or medical care. Participants accepted are those who have physical, mental, or emotional impairments. These  limitations may include the need for long-term and indefinite care that could  substantially impede their ability to live independently.  Persons with net incomes between 100% and 200% of the Federal  Poverty Level (FPL) are expected to pay a fee to contribute toward these  personal care services. Participants are not discriminated based on age, religion, race or gender. Benefits These persons could  benefit by living in a family-style environment as opposed to the traditional  nursing home which usually results in substantial functional limitations.The physical and emotional benefits include participating in normal life activities,  self-care; receptive and expressive language; learning; increased mobility;  self-direction;encouragement for independent living and economic  self-sufficiency. These benefits address the individual’s need for assistance that is lifelong or of extended duration. Activities are individually planned  and coordinated.
