Class of Housing/Services: short-term homeless prevention and re-housing assistance.

General Information:
Homeless Prevention Ninth District Opportunity provides qualifying households with short-term homeless prevention and re-housing assistance. Candidates for this program will be required to participate in an in-depth case management plan. A minimum of the following items will be required to verify eligibility for this type of assistance: - Verification of social security numbers for all household members - Verification of all household income for the past 30 days - Verification of homelessness or danger of eviction - Additional items will be required NDO's housing programs were designed to assist households that are currently homeless or in imminent danger of becoming homeless with the establishment of long-term housing stability through limited financial assistance and intense case management services. Enrollment in the agency's housing programs are limited to applicants that meet a specific criteria and are willing to participate in a case management plan. Applicants may be required to participate in job search activities or other individualized case management activities, requiring multiple office visits each month.
