Class of Housing/Services: Homeless Case Management, Food Pantry, Emergency Services

General Information:
Provides intensive case management to a maximum of 30 chronically homeless, dually diagnosed persons with mental illness and/or substance abuse problems through prevention, diagnosis and treatment services as well as to address the community risk factors that contribute to these illnesses. Office hours: 9am - 5pm, M - F. Food Pantry M, TH and F, 10am- Noon and 1pm-3pm.
