General Information:
Our case managers play a critical role in the success of our programs. They provide guidance, support, and encouragement for homeless families and individuals. They are a source of stability at a crucial juncture in clients’ lives. Our Passages Case Management program was started in 1997. This program provides a range of services to all homeless adults and families in Somerville, designed to move them from the streets and shelters into permanent housing and self-sufficiency. Every agency in the city that works with homeless individuals and families refer their clients to Passages, which then coordinates all referrals for services and manages follow-up. We worked with over 323 people in 2012 alone, 64% of whom moved into permanent housing. The needs of our clients range from basic needs such as housing and shelter, life skills training like financial literacy, GED Preparedness, and job readiness to referrals for crisis care, medical assistance, transportation and educational accommodations.
