Class of Housing/Services: shelter and services for families.

General Information:
Short Term Rapid Rehousing Rapid scattered site housing placement that includes security deposits, rent subsidy and home based wrap-around housing, economic and Mental Wellness supportive services.  Qualified participants are chronically homeless. Enable  families and individuals to rapidly obtain safe, affordable housing, increased income, and other resources to sustain long-term housing independently. Service timelines range from 2 to 24 months. Long Term Rapid Re-housing Rapid scattered site housing placement that includes security deposits, rent subsidy and home based wrap-around housing, economic and Mental Wellness supportive services.  Qualified participants are chronically homeless and at least one individual is diagnosed with a long-term disability. Households exit when they are able to maintain housing stability independently. SOAR Project SSI/SSDI Outreach, Access, and Recovery (SOAR) is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) and is a national program designed to increase access to the disability income benefit programs administered by the Social Security Administration (SSA) for eligible adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder. Our SSI Ohio Project specialist helps people successfully apply for SSI/SSDI and other benefits using the SOAR methodology.
