General Information:
Recovery Home Program: The Recovery Home program is a long-term, 12-18 month, mens residential program. It offers the participant a structured, disciplined environment where he learns about the underlying causes that drive his drug and alcohol abuse. The participant will also learn to use the tools of recovery to remain clean and sober. To be considered for entry into the program the following eligibility requirements must be met: Homeless within Polk County     Substance abuse problem     Referred through the Polk County Continuum of Care Coordinated Entry system* Permanent Supportive Housing Program: Our permanent supportive housing program is permanent housing with indefinite leasing paired with substance abuse supportive services to assist homeless men achieve housing stability. This program offers reasonable monthly rental rates for men actively involved in a program of recovery. The structures are typically shared 3 or 4 bedroom single-family dwellings. Participants are to abstain from all alcohol and drug usage. ***NO VACANCIES*** (HUD) Housing First Permanent Supportive Housing Program: We have low-barrier, one-bedroom apartments available for the chronically homeless, single male person with a diagnosable substance use disorder. All clients for this program must be referred through the Polk County Continuum of Care Coordinated Entry system*.
